I have questions about Form W-2, who should I contact?
W-2 tax statements were sent February 2, 2009 and are available online beginning February 15, 2009. To access your W-2 online, go to the Payroll website at www.payroll.ucla.edu; click on Your Paycheck and At Your Service Online:

  • If you have never used At Your Service Online or the system doesn't recognize your social security number, contact Alan Ackereisen in the Payroll office at 310-794-8754 or send an email to aackerei@finance.ucla.edu to reset your password so you can access the system.
  • Requests for duplicate W-2 forms should be sent to aackerei@finance.ucla.edu.

I have questions about my W-2 form (Wage and Tax Statement) issued by UCLA, who should I contact?

U.S. Resident Employees:
Form W-2 is filed with the Internal Revenue Service to report the following withholding information for UCLA employees: 

  • Compensation - income and wages paid by UCLA
  • Medicare 
  • Social Security 

Nonresident Alien Employees:
Nonresident alien employees claiming U.S. Tax Treaty benefits will receive both Form W-2 and Form 1042-S.  

  • Form W-2 will reflect the nonresident alien employees' federal compensation 
    that is not covered under a U.S. Tax Treaty, their state compensation, and the 
    related federal and state tax withholding for such amounts.  
  • Form 1042-S will reflect the nonresident alien employees' federal compensation 
    covered under a tax treaty as well as the related federal tax withholding.  Click on Form 1042-S to find out more information about this form.
