Request for Taxpayer Identification Number & Certification

 

                                         
        If my department should receive a request for the University's Taxpayer Identification
  Number (EIN), how should I respond to a request on behalf of the University?
   
         Form W-9 (Request for Taxpayer Identification Number and Certification) is the standard form published
   by the Internal Revenue Service (IRS) used by payers to request TIN information.  
   A person who is required to file an information return with the IRS must get the University's correct TIN to report
   such information.
  • In general, requests for UCLA's Taxpayer Identification Number (TIN) are made by payers
    in order for them to properly report information in connection with miscellaneous payments 
    made to the University (interest, dividends, rents, royalties, etc.) or by donors to enable
    them to claim tax deductions for gifts and donations made to the University.

   How to respond: 

  • Departments can respond to the requesting party directly by retrieving a completed Form W-9 which indicates UCLA's taxpayer ID number.  
  • Please sign, date and send the form to the requesting party.
  • Please note that this form indicates UCLA's general Employer Identification Number (EIN) 95-6006143.  Click on Taxpayer ID Numbers for more information about other taxpayer ID numbers utilized by UCLA such as the UCLA Foundation and the tax IDs for patient billing purposes.
